New Kareo Website

Today, we launched a new Kareo website located at http://www.kareo.com/

We had two goals for the new Kareo website.  First, to support the Kareo brand’s recent extension to small and medium sized physician practice customers.  Second, to introduce the Kareo Billing Partners Program, a program we have launched to help connect physicians with successful third-party billing services.

The new website has many new features, including:

- New home page that explains the benefits of the Kareo service to everyone associated with a medical practice.

- Tour videos that provide a guided demonstration of the features that matter most to different types of users.

- Feature videos that showcase specific features and the benefits they offer all customers.

- Customer stories that will soon showcase stories from customers of how Kareo helped their businesses.

- Streamlined sign up process that helps new customers get signed up with Kareo quickly.

- Partners section to introduce and recruit participants to the Kareo Billing Partners Program.

Read this Book: The Physician Billing Process

There is so much to learn about the business of healthcare.  One of the ways I try to keep up is to spend a ridiculous amount of time reading anything and everything I can get my hands on.  In fact, I just finished reading arguably the best book I’ve ever read on the subject.  Let me tell you why you need to buy and read this book today.

Thephysicianbillingprocess The book is entitled “The Physician Billing Process: Avoiding Potholes in the Road to Getting Paid” written by Deborah Walker, Sara Larch, and Elizabeth Woodcock, three accomplished practice management consultants and speakers.  The book is published by the Medical Group Management Association (MGMA) and Medical Economics magazine has hailed the book as “A bible on the science of reimbursement”.  One of the reasons I originally picked up the book was because I recognized the name of one of the authors, Elizabeth Woodcock, who had given a benchmarking seminar I attended in September at the Healthcare Billing and Management Association (HBMA) fall conference in Las Vegas.

Unlike other practice management books, this one focuses exclusively on how you can manage your practice to ensure you maximize reimbursement and get paid for your services.  Most of the book analyzes each stage of the revenue cycle process including previsit, patient check-in, charge capture, claims, payment posting, insurance follow-up, denial management, and patient collections.  The remainder covers special topics including rightsizing your staff, using technology, in-house vs. outsourced billing, reporting, and compliance.

Here are a few of the topics I enjoyed most:

First, each chapter includes performance benchmarks collected by the authors from a study of real practices.  For example, the authors say the optimal staffing level for all back-office billing functions is approximately one staff member per 10,000 claims.

Second, a major theme throughout the book is getting things right the first time.  To drive home this point, chapter 1 provides a worksheet that helps you calculate how much denied claims cost you and shows you how some practices spend more money resolving denials than they collect by resubmitting the claim. 

Finally, chapter 13 provides a number of sample reports you can use to manage and analyze your revenue cycle.  I’ve seen a lot of report – from customers, experts, even competitors – but I haven’t seen a set of reports that are as concise, yet insightful as those the authors have provided in this book.

This book has something to offer for everyone involved in a physician’s revenue cycle.  If you are new to billing, this book will give you a fantastic introduction to the process.  If you are veteran guru, use this book to compare your business to industry best practices and performance benchmarks.  You can pick up a copy of this book at the MGMA store by clicking here for the catalog page for The Physician Billing Process.  It costs $65.00 for members and $105.00 for non-members.  Buy it for yourself, or better yet, buy it for your entire staff.  That’s what I did!

CMS Announces Contingency Plan for NPI Deadline

The Centers for Medicare & Medicaid Services (CMS) announced today a contingency plan that extends the compliance date for the National Provider Identifier (NPI) to May 23, 2008 for covered entities that will not meet the May 23, 2007 deadline, but whom are making a “good faith effort” to do so.

Basically, CMS is saying that they now realize that many covered entities will not be able to fully comply with the NPI standard by the May 23, 2007 deadline.  So they will not take any enforcement action on non-compliant entities if they can show CMS they are making a “good faith effort” to comply with the standards.  Some of the examples of good faith efforts include:

- For a health care provider, having obtained an NPI and having the ability to use it on HIPAA transactions.

- For a health plan, concerted efforts in advance of the May 23, 2007 deadline and continued efforts afterwards to conduct outreach and make testing opportunities available to its provider community.

- For other “trading partners”, increased external testing with other trading partners.

We don’t think you can interpret this announcement as a blanket extension of the NPI deadline.  Instead, we think CMS is basically saying, hey, let’s not kid ourselves, not everyone is ready for full NPI compliance and we need to give organizations a backup plan who are genuinely doing their best to implement the NPI standard.

We recommend Kareo customers continue at full throttle on their NPI transition.  We are ready for NPI today.  You can enter NPI numbers for practices, providers, service locations, and referring physicians in Kareo.  You can print out the new CMS 1500 form with NPI support.  And just a few weeks ago, we started sending NPIs in all outbound electronic claims.  So we recommend you take advantage of Kareo’s NPI support and make every effort to comply with the original May 23, 2007 deadline.

Kareo Sending NPIs in All Electronic Claims

A few weeks ago, we started sending National Provider Identifiers (NPIs) in all electronic claims to all payers, provided you have entered your NPIs in Kareo.

There are four areas where you enter NPI numbers in Kareo:

1. Practices - You can setup a group NPI number on the practice record by selecting the menu item Settings > Practice Information.

2. Providers – You can setup a provider NPI number on the provider record by selecting the menu item Settings > Providers.

3. Referring Physicians – You can setup a referring physician NPI number on the referring physician record by selecting the menu item Settings > Other Lists > Find Referring Physicians.

4. Service Locations – You can setup a service location NPI number on the service location record by selecting the menu item Settings > Service Locations.

We have taken every effort to minimize the risk of problems associated with sending NPI numbers in electronic claims.  However, as with any changes in your provider’s insurance claim stream, you may experience problems if your numbers are not setup properly or specific payers have difficulty accepting NPI numbers from Kareo.  You should be extremely vigilant in monitoring the system for rejections and initiating troubleshooting immediately with our customer support team if you notice any problems.

Managing Copays with Kareo

Some Kareo users have written our support team requesting help with the new process for managing copays using the January 2007 release.  Here is some background information on the changes and a link to some new documentation covering the subject.

With the January 2007 release, Kareo introduced some important changes to posting payments in encounters, tracking copay amounts due, applying patient copays when approving encounters, and tracking patient versus insurance responsibility.  These new changes enable schedulers to accept copays and print receipts from the appointment calendar, and also enable billers to track and bill for missed copays concurrently with the insurance billing process.

Kareo Live and Recorded Webinars

We are proud to announce the launch of some exciting new training options for Kareo users.  The Kareo Help & Support site now features a training library, including live webinars, pre-recorded webinars, and self-paced training videos. 

The live webinars are web-based video meetings along with a live conference call.  We plan to host between 5-10 live webinars per week.  Initially, the live webinars will focus on Kareo's core functionality.  But once we compile a complete library of pre-recorded webinars covering core functionality, the live webinars will highlight new feature releases and advanced topics, such as real-time patient eligibility, kFax, business rules, and more.  We encourage Kareo users to RSVP for the live webinars by emailing support@kareo.com.  For more information, please visit Kareo's live webinar schedule.

We are also archiving recordings of our live webinars for Kareo users that are unable to attend the live webinars.  These pre-recorded webinars can be viewed at your own pace and they are a good option for users interested in learning an entire category of functionality.  Our plan is to compile a library of 7-10 pre-recorded webinars within the next week that we think will provide a valuable training option we can post quickly.  So far this week, we have posted recorded webinars covering setting up your company, setting up a practice, and using the new payment posting screen.  For a complete list of pre-recorded webinars now, please visit the Kareo pre-recorded webinar library now.

Finally, we also plan to build a high-quality library of self-paced training videos.  Unlike the live or pre-recorded webinars, the self-paced training videos will be organized into 20+ videos, each about 10-15 minutes in length.  The self-paced training videos include a table of contents and you can skip to specific topics that catch your interest.  We have already posted a few self-paced training videos, but some of these videos need to be updated based upon the new functionality in the Kareo January 2007 release.  We recommend checking back on this section in 3-4 weeks.

Sneak Peek: Project kFax

Today is an exciting day for Kareo users.  In this post, I’d like to give you a sneak peek of kFax, Kareo’s upcoming fax service that integrates the worlds of telephony, document management, and revenue cycle management.

Most third-party medical billing companies still receive mountains of paperwork from clients via standard fax machine, express mail, or courier services.  Shuttling around all of that paper is expensive, time-consuming, and subjects your patients’ private healthcare information to unnecessary risks.

So we had an idea.  Since there is nearly universal adoption of fax machines in every medical office, we thought, what if we could use any standard fax machine as a data capture tool for Kareo?

Introducing kFax!  Kareo’s innovative new service gives you the power to use any standard fax machine to capture and scan paperwork directly into Kareo’s integrated document management system. 

Here is how it works.  Kareo assigns each medical practice a kFax number (e.g. 949-555-1234).  You or your clients can use any standard fax machine to fax paperwork to the kFax number assigned to one of the medical practices associated with your Kareo subscription.  These faxes are received by Kareo’s fax servers, converted into scanned documents, and automatically loaded into a document inbox associated with the corresponding medical practice.  Then you can open and manipulate the faxed document just as if you scanned it directly into Kareo applications from your own desktop.  It’s just that simple!

Using the kFax service with Kareo’s other document management and medical billing tools can transform your data entry process and provide a greater level of efficiency than ever possible before.  Once your clients kFax their paperwork into Kareo, you can display the scanned document on screen, side-by-side with your data entry records.  After you complete your data entry, you can attach individual pages of a kFax batch directly to the corresponding records in order to cross-reference your Kareo records with the originating document.  This means you can complete the entire process -– receiving paperwork, entering records, and archiving your documents –- without ever printing, touching, or shredding paper.  Kareo’s new kFax service truly is the real-world manifestation of the paperless medical billing office, and it’s easy!

Kareo’s new kFax service is currently in testing with a couple of customers.  The kFax service will be rolled out and generally available to other customers within the next couple weeks.  Stay tuned for more information about this exciting development!

Kareo and the National Provider Identifier (NPI)

There are too many numbering schemes in the U.S. to identify healthcare providers.  Most physicians today have a Social Security Number, Employer Identification Number, Drug Enforcement Administration number, State License Number, Medicare Provider Number, Medicaid Provider Number, Blue Cross Provider Number, CHAMPUS Provider Number, State Industrial Accident Provider Number, and on, and on, and on.   Keeping track of all of these numbers causes major problems in healthcare reimbursement.  But that’s about to change.

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) mandated that the Secretary of Health and Human Services adopt a standard unique identifier for health care providers.  It took nearly 10 years, but the National Provider Identifier (NPI) has finally arrived and the industry is fast preparing for the May 23, 2007 mandatory compliance date.

The NPI numbers are assigned by the Centers for Medicare and Medicaid Services.  The NPI is a numeric 10-digit identifier, consisting of 9 numbers plus a check-digit in the 10th position.  Healthcare providers can get more information at the National Provider Identifier web site or apply online at the National Plan and Provider Enumeration System web site.  Medicare recommends that you apply for an NPI immediately, but no later than November 2006.

There are certain timelines you need to know:

Jan 3, 2006 – Oct 1, 2006 - Medicare will accept claims with an NPI, but an existing legacy Medicare number must also be on the claim.  Medicare will reject, as unprocessable, any claim that includes only an NPI.

Oct 2, 2006 – May 22, 2007 – Medicare will accept an existing legacy Medicare billing number and/or an NPI on claims.  If there is any issue with the provider’s NPI and no Medicare legacy identifier is submitted, the provider may not be paid for the claim.

May 23, 2007 – Forward – Medicare will only accept NPI numbers.  Small health plans have an additional year to be NPI compliant.

Kareo is doing its part to comply with the NPI standards.  You can already setup an NPI as a provider number for a provider in your practice settings.  The NPI will be sent along with electronic claims today.  Also, Kareo will be supporting a new version of the CMS 1500 form designed to support NPI numbers by October 1, 2006.  If you have any questions about Kareo’s support for NPI, you can email support@kareo.com.
